MHW630 Grand Canyon University DSM In Service Scenario Analysis Paper

Topic 6: In-Service Scenario Analysis

Directions: In 750-1,000 words, select one of the scenarios below to write an essay that answers the reflection questions. Use a minimum of three scholarly references to support your analysis.

Scenario 1:

Marcie is a new client and a 22-year-old female who works as a receptionist in a doctor’s office. She has just revealed to you during the intake session that she has recently lost interest in most activities, has been sleeping a great deal yet feels tired all the time, and sometimes wishes she could cease to exist. She mentioned feeling as though she has been “on an emotional roller coaster” during the past year, throughout her on-again/off-again relationship with a 35-year-old married man. The last breakup with him seemed final, and Marcie has felt herself sinking deeper and deeper into depression ever since. When probed further about suicidal ideations, Marcie admitted that she has considered killing herself, although she is uncertain whether or not she would actually do it.

Scenario 2:

A managed care clinician completed a utilization review and has just denied authorization for you to continue treating a client. You believe that the client could benefit from four additional sessions. At the same time, you understand that the managed care clinician must apply criteria of medical necessity to justify continued treatment, and you are aware that many of your agency’s clients have problems much more severe than your client’s problems.

Reflection Questions:

  • Does the primary diagnosis via the current DSM play a factor in what you would do as an administrator of the facility?
  • What steps would ensure the safety of the client within a community/mental health care setting?
  • Would communication be a concern within the agency? How would you, as the administrator address training to ensure privacy of the client? What could be included within the informed consent process to ensure emergency notification for the client?
  • Make a list of all of the ways that you could try to ensure that the managed care system does not prevent you from giving your client the type and duration of treatment services that he or she needs.
  • Categorize each of the items from question 4 into two groups reflecting those that would be considered professionally appropriate and ethical, and those that would be considered a violation of professional ethical standards.

Introduction:

In the field of medical education, designing and conducting lectures, evaluating student performance, and providing feedback through examinations and assignments are important responsibilities of a medical professor. In this assignment, we will be analyzing two scenarios and answering reflection questions related to them. The first scenario is about a 22-year-old female client who has been suffering from depression due to an on-again/off-again relationship with a married man. The second scenario is about a managed care clinician denying authorization for further treatment of a client. We will be analyzing the scenarios and answering the reflection questions.

1. Does the primary diagnosis via the current DSM play a factor in what you would do as an administrator of the facility?

As an administrator of the facility, the primary diagnosis via the current DSM (Diagnostic and Statistical Manual of Mental Disorders) would definitely play a factor in the decision-making process. The DSM serves as a comprehensive guide that helps clinicians in the diagnosis and treatment of mental disorders. It provides a common language that enables clinicians to communicate with each other about the diagnosis, prognosis, and treatment of mental disorders. Therefore, as an administrator, it is important to ensure that the diagnosis is accurate and based on the current DSM criteria. This would ensure that the client receives the appropriate treatment that is specific to their diagnosis.

2. What steps would ensure the safety of the client within a community/mental health care setting?

Ensuring the safety of the client is paramount in a community/mental health care setting. The following steps can help in ensuring the safety of the client:

– Conducting a thorough assessment of the client’s mental health history, including suicidal ideation, previous suicide attempts, and other risk factors.
– Developing a safety plan that includes emergency contacts, crisis hotlines, and means restriction (such as limiting access to lethal means).
– Providing ongoing monitoring and support to the client, including regular check-ins and follow-up appointments.
– Coordinating care with other providers (such as primary care physicians or psychiatrists) to ensure that the client is receiving comprehensive and coordinated care.
– Educating the client and their family/support system about warning signs and risk factors for suicide.
– Providing suicide prevention training to all staff members in the community/mental health care setting.

3. Would communication be a concern within the agency? How would you, as the administrator address training to ensure privacy of the client? What could be included within the informed consent process to ensure emergency notification for the client?

Communication is a critical factor in ensuring effective care in a community/mental health care setting. It is important to ensure that communication is clear, consistent, and confidential. As an administrator, I would address training to ensure the privacy of the client by providing regular training to all staff members on the importance of maintaining confidentiality and privacy. This would include regular reminders about the importance of HIPAA regulations and privacy laws. Within the informed consent process, we could include emergency notification information that would allow us to contact the client or their designated emergency contact in the event of an emergency.

4. Make a list of all of the ways that you could try to ensure that the managed care system does not prevent you from giving your client the type and duration of treatment services that he or she needs.

The following are ways in which we could ensure that the managed care system does not prevent us from providing the appropriate type and duration of treatment services to our clients:

– Ensuring that our documentation is accurate and thorough, and includes all necessary information to support medical necessity.
– Utilizing evidence-based practices and interventions that are cost-effective and efficient.
– Building relationships with managed care organizations and advocating for our clients when necessary.
– Collaborating with other service providers, such as primary care physicians and social workers, to ensure that clients receive the comprehensive care that they need.
– Providing regular updates and progress reports to the managed care organization to demonstrate the effectiveness and necessity of our services.

5. Categorize each of the items from question 4 into two groups reflecting those that would be considered professionally appropriate and ethical, and those that would be considered a violation of professional ethical standards.

Professionally appropriate and ethical:
– Ensuring that our documentation is accurate and thorough, and includes all necessary information to support medical necessity.
– Utilizing evidence-based practices and interventions that are cost-effective and efficient.
– Building relationships with managed care organizations and advocating for our clients when necessary.
– Collaborating with other service providers, such as primary care physicians and social workers, to ensure that clients receive the comprehensive care that they need.
– Providing regular updates and progress reports to the managed care organization to demonstrate the effectiveness and necessity of our services.

Violation of professional ethical standards:
None of the items listed in question 4 would violate professional ethical standards.
