In this third assessment, you will use the data analysis from the previous two assessments. You will fill in the necessary content under each of the prompts (headings and subheadings) You will draft this initial CHAP
content to organize your research and set your top 3 population health S.M.A.R.T. goals, and corresponding proposed community intervention programs to meet those goals. Once your draft is completed, submitted for a grade and returned, then you will use the returned copy to populate the final Assessment 4, which is the final CHAP for publication. Note that you will have limited space in your final CHAP PowerPoint document for publication. You will want your writing to be clear, concise and succinct in this document, so that it is easier to transition to the final document.
[Since this is your first experience writing a CHAP, it is highly recommended that you review the example Exemplar draft in the assessment instructions. Do not copy any information from the Exemplar. All project work must be original from the learner.]

Key Stakeholders Recognition
[Identify and acknowledge the key stakeholders involved in your project in 1 short paragraph. Include their names, titles, and the organizations they represent.]
CHAP Methods
[Describe the methods you used or will use to conduct your CHAP. This section, usually 1-2 paragraphs, should detail the processes and techniques used to gather and analyze data, and to engage with stakeholders.]
CHAP Resources
[List and briefly describe the resources that will be used to implement your CHAP. Resources may include personnel, facilities, equipment, and supplies.]
CHAP Executive Summary
[Write an executive summary of your CHAP. This summary, usually 1-2 paragraphs, should encapsulate the main points and findings of your plan. Readers should get an overall idea of your CHAP from this summary.]
CHAP Community Scope
[Describe the community in which your CHAP will be implemented. This section, usually 2-3 paragraphs, should include relevant demographic, socioeconomic, health status data or any other determinants of population health being addressed in your CHAP.]
