Four years earlier the AACN ethics work group developed a call-to-action plan titled The Four A’s to Rise Above Moral Distress (2004). Nurses use the Four A’s plan as a guide to identify and analyze moral distress:

  • Affirm your distress and commitment to take care of yourself and address moral distress.
  • Assess sources of your moral distress to prepare for an action plan.
  • Act to implement strategies for changes to preserve your integrity and authenticity.

Preventing moral distress requires nurses to recognize the at-risk dynamics and issues. An environment of good communication and respect for others is essential for decreasing the likelihood of experiencing moral distress.

Therefore, while I acknowledge the potential benefits of assigning grades on a curve, I lean towards an approach that combines both relative and absolute grading. This would involve considering individual performance and mastery of the material alongside comparison to peers, promoting a balanced evaluation system that accounts for both relative and absolute achievements.

2. Three fair and accurate clinical evaluation strategies:
a) Direct Observation: This strategy involves directly observing students during clinical practice, allowing for comprehensive assessment of their cognitive, psychomotor, and affective domains of learning. This approach provides an opportunity to evaluate students’ clinical competence, professionalism, communication skills, critical thinking abilities, and patient interaction. Direct observation ensures an accurate assessment by directly witnessing students’ performance and behavior in real-life scenarios.

b) Objective Structured Clinical Examination (OSCE): OSCE is a standardized evaluation method that consists of multiple stations, each simulating specific clinical scenarios. This strategy assesses students’ abilities to apply theoretical knowledge, perform clinical skills, and demonstrate effective communication and patient-centered care. OSCEs are designed to eliminate evaluator biases and provide a fair and consistent evaluation across multiple students.

c) Case Studies and Reflections: This strategy requires students to analyze complex clinical cases and reflect on their decision-making processes, ethical considerations, and lessons learned. Case studies encourage critical thinking and application of knowledge to solve clinical problems. Reflective assignments provide insight into students’ affective domain, allowing them to describe their emotional responses, ethical dilemmas encountered, and strategies for addressing moral distress. – Act to implement strategies: The nurse actively seeks strategies to effect change by collaborating with the healthcare team, promoting ethical dialogue, advocating for patient rights, and participating in efforts to improve resource allocation and healthcare policies. Implementing such strategies aims to preserve the nurse’s integrity and authenticity while addressing the moral distress and striving for positive change.

4. Ethical considerations of social media use in nursing:
Social media use in nursing can potentially violate ethical practices in several ways. These include breaching patient confidentiality by sharing patient information or images without proper consent, engaging in unprofessional or derogatory discussions about colleagues or patients, and providing medical advice without proper qualifications or authorization. Such violations can compromise patient trust, confidentiality, and professional integrity.

On the other hand, ethical social media use can be beneficial to healthcare professionals and their patients. It can facilitate professional networking, knowledge sharing, and interdisciplinary collaboration. By engaging in online communities and platforms, healthcare professionals can access the latest research, exchange evidence-based practices, and contribute to the advancement of medical knowledge. Ethical social media use allows healthcare professionals to educate and disseminate accurate health information to the public, thereby promoting health literacy and patient empowerment. However, it is crucial for healthcare professionals to adhere to ethical guidelines, maintain patient privacy, and separate personal and professional identities online to ensure responsible and beneficial use of social media.
